Red flags (4)

  • Omphalitis can progress rapidly to systemic infection and death, with an estimated mortality of 7% to 15%
  • Lethargy, poor feeding, fever and irritability in the neonate suggest sepsis and a worse prognosis
  • Rapidly spreading abdominal wall redness, or gas in the surrounding tissue, means necrotising fasciitis and needs an acute surgical opinion
  • Omphalitis with recurrent infections should prompt a workup for leucocyte adhesion deficiency

Signs — what you find (4)

  • A benign vascular nodule at the base of the umbilicus that appears after the cord detaches, typically about 5 mm across [umbilical lump]
  • Moist, red and strawberry-like, because of the prominent capillaries on its surface
  • Soft, velvety texture and pinkish colour are what distinguish it from infection, and it typically appears after the first week of life
  • The friable tissue causes serous or serosanguinous discharge and bleeds easily with trauma, which is why it is mistaken for an umbilical infection [bleeding tendency]

If not this — what else fits (6)

  • Omphalitis is the infection to exclude: local tenderness, redness and induration of the umbilicus and surrounding tissue, with purulent discharge or bleeding from the stump
  • A foul-smelling discharge raises the suspicion of anaerobic infection
  • A patent urachus connects the bladder to the umbilicus and causes persistent drainage that can be mistaken for infection
  • An umbilical polyp is a firm mass of urachal embryological remnant; it needs surgical excision but does not cause infection
  • Funisitis is inflammation of the cord alone - a wet, foul-smelling stump without surrounding cellulitis
  • Cord separation is normally within the first few weeks; delayed means more than 3 weeks, and warrants evaluation if there is also skin infection or a persistent umbilical anomaly

ARZOL may be applied direct to mucous membranes and other moist surfaces. In the case of dry skin, the applicator tip should be dipped in water immediately before use. Apply carefully to the area to be treated. - Neither document states a number of applications or an interval - apply once and review

Paediatric dose

Neither document states a strength, a frequency, a number of applications or a maximum for a neonate. The disease article gives the treatment without a dose; the product label gives the application without a schedule. No number is invented here. What the label does state about neonates is in the cautions and is the point: keep the contact extremely brief.

Why

Indication, from StatPearls: a granuloma is a benign overgrowth of blood vessels that can appear once the umbilical cord stump separates, and standard treatment is chemical cautery with silver nitrate, applied carefully to avoid injuring the surrounding skin. Application, from the ARZOL silver nitrate applicator label: its licensed skin use covers cauterising wounds and sluggish ulcers, and removing granulation tissue and warts, with the application steps given above. No Egyptian product corresponds to it - the register lists no silver nitrate at all - so no local brand or price is shown, on the same basis as the oxygen row in croup.

Cautions
  • NEONATAL WARNING FROM THE LABEL - where a silver nitrate applicator touches a newborn, as it does on the umbilicus, and wherever the skin is thin and delicate, the contact must be extremely short. This is the single most important line on the card.
  • IT IS A CAUSTIC - silver nitrate is caustic, and used wrongly it can burn the skin chemically. It also mixes with body fluid, and that mixture is caustic in its own right; leave it sitting and the tissue is damaged. Dry the surrounding skin, protect it, and wipe away any run-off at once.
  • THE DISEASE ARTICLE'S OWN QUALIFIER - apply it carefully, so as to AVOID injuring the skin around the granuloma.
  • DO NOT PUT IT ANYWHERE NEAR THE EYES.
  • REPEATED USE - applying it over and over to a large area can bring on argyria: a slate-blue staining of the skin and the mucous membranes. A granuloma that has not gone after two or three applications should be referred, not cauterised again and again.
  • REGULATORY STATUS, STATED HONESTLY - this product is marketed as an unapproved drug, and the FDA has not reviewed it for safety, efficacy, or its labelling. It is cited here only because it is the sole opened document that describes how to apply the product; the indication itself comes from StatPearls.
  • No Egyptian product is listed for silver nitrate. The applicators are hospital or clinic stock, not a pharmacy prescription the family fills.

Why

The whole safety of umbilical granuloma care is in telling a granuloma from an infected umbilicus. Omphalitis is a neonatal emergency and looks, at first, like a wet navel.

Cautions
  • RED FLAG - OMPHALITIS: infection of the umbilicus and the tissue around it, seen most often in newborns, and classed as a medical emergency because it can run on quickly to systemic infection, to sepsis, and to death. Do not cauterise an infected umbilicus - admit it.
  • WHAT TO LOOK FOR - in a newborn: tenderness, redness and induration of the umbilicus and the tissue around it. Pus or bleeding from the cord stump. A foul smell should make you think of anaerobes. And where the baby is lethargic, feeding poorly, febrile or irritable, that points to sepsis and to a worse outlook.
  • HOW A GRANULOMA DIFFERS - a granuloma is easily mistaken for infection, because the friable tissue weeps serous or serosanguinous fluid and bleeds at the slightest knock. It usually turns up after the first week of a baby's life, and what tells it apart is the texture and the colour: soft and velvety, and pink.
  • SURGICAL EMERGENCY - where redness of the abdominal wall is spreading fast, or there is gas in the tissue around it, think necrotizing fasciitis.
  • NOT A TOPICAL PROBLEM - omphalitis is not treated with silver nitrate, nor with any other topical. It needs broad-spectrum antibiotics given parenterally, and that is an inpatient decision.
  • NOT EVERY UMBILICAL LUMP IS A GRANULOMA - an umbilical polyp is a firm mass, a leftover of the urachus in the embryo; it often has to be excised surgically, and it does not cause infection. A firm lump, or one that will not resolve, is a surgical referral. And an umbilicus that keeps draining may be a patent urachus, with the bladder communicating directly with the umbilicus.
  • DELAYED CORD SEPARATION (beyond 3 weeks) is its own signal - the umbilical cord article says to look further in a newborn whose cord separated late and who also has a skin infection, or a persistent umbilical anomaly such as a urachal remnant.
