Symptoms — what the patient reports (3)

  • Stiffness, discomfort, or worsening pain on the palm side of the finger during bending, often with a painful click [finger pain · joint pain]
  • The finger can lock while straightening out, or get stuck in a bent position [locking]
  • Onset can be gradual or sudden, and symptoms often interfere with work

Signs — what you find (2)

  • A tender lump or swelling can be felt at the base of the finger on the palm crease [skin nodule]
  • The digit may be stuck in a flexed position, and moving it can cause pain or a snapping sensation [finger pain · joint pain · locking]

Tests (4)

  • Diagnosis is clinical - a finger that locks in flexion, clicks painfully, and catches on extension
  • Ultrasound is the preferred imaging test for this condition
  • Ultrasound can show the catching and clicking of the tendon dynamically during motion
  • Plain x-rays can exclude an occult fracture, generally making MRI or CT unnecessary

METHYLPREDNISOLONE

4-30 mg methylprednisolone acetate (with 1% lidocaine), or 5-10 mg triamcinolone acetonide using the 10 mg/mL preparation, injected into the flexor tendon sheath at the A1 pulley; single injection, may repeat once after 6-12 weeks - Single local injection

Paediatric dose

Congenital trigger thumb in children requires pediatric orthopedic evaluation for splinting or surgical A1 release.

Why

Corticosteroid sheath injection is the definitive non-surgical treatment of choice (~60-80% resolution rate), combined with nocturnal extension splinting.

Cautions
  • Avoid intratendinous injection to prevent flexor tendon weakening or rupture.
  • Transient elevation of blood glucose may occur in diabetic patients following corticosteroid injection.
  • If 2 corticosteroid injections fail to resolve triggering, refer for surgical percutaneous or open A1 pulley release.
  • Use the 10 mg/mL triamcinolone preparation for a short tendon sheath, not the 40 mg/mL - the stronger strength is not intended for this site.

Adult dose and duration

250-500 mg twice daily with food PRN for local inflammation and discomfort x 7-14 days

Paediatric dose

Not recommended under 16 years for this indication - the product label restricts paediatric naproxen to juvenile idiopathic arthritis (10 mg/kg/day in two divided doses, child over 5 years). Use paracetamol or ibuprofen instead.

Why

Adjunct NSAID added for symptomatic pain and inflammation relief in trigger finger; it does not mechanically release the constricted flexor tendon sheath, so it does not replace splinting, corticosteroid injection, or surgical release when needed.

Cautions
  • Oral NSAIDs provide symptomatic relief but do not mechanically unlock flexor tendon sheath.
  • Take with food to reduce gastric irritation.
  • The weight fields are empty on purpose. The note already says naproxen is not for under-16s in this indication, but the numbers stayed behind it and the calculator went on drawing a dose for a 10 kg child. A withdrawn drug must not still have a working dose card.
