Symptoms — what the patient reports (12)

  • Adult tapeworm infection is usually silent or causes only mild trouble
  • The typical picture is itching around the anus from escaping worm segments, plus mild belly pain [abdominal pain · itching]
  • Most people first realise when they notice worm segments passed in the stool
  • Rarely a segment is brought back up or inhaled
  • Other complaints are nausea, appetite change in either direction, weakness and weight loss [nausea · weight loss]
  • Less often headache, constipation, dizziness or diarrhoea [constipation · diarrhoea · dizziness · headache]
  • Children tend to have more symptoms than adults do
  • The fish tapeworm reaches 2 to 15 m yet still causes mild illness or none at all
  • One in five with the fish tapeworm get diarrhoea, belly pain, tiredness, constipation or pernicious anaemia [abdominal pain · anaemia · constipation · diarrhoea · fatigue]
  • Bloating, a sore tongue, sore gums and a change in appetite are also reported [abdominal distension]
  • Brain cysts can imitate almost any neurological illness [cyst]
  • Reported features are fits, focal weakness, raised pressure in the head, decline in thinking, headache and abnormal movements [headache · seizures]

Signs — what you find (8)

  • Worm segments may be seen in the stool or brought back up
  • Cysts survive only where the immune system cannot reach, chiefly the brain and the eye [cyst]
  • Cysts sit most often in muscle, under the skin, in the eye and in the brain, but only brain cysts carry real morbidity [cyst]
  • Where the brain cysts sit sets the picture: inside brain tissue means fits and headache, in the fluid spaces means hydrocephalus [cyst · headache · seizures]
  • Rare complications are blockage of bowel, bile duct or pancreatic duct needing surgery
  • The fish tapeworm can likewise obstruct the bowel or inflame the gallbladder
  • A heavy or long-standing fish tapeworm can cause megaloblastic anaemia, taking up to 80% of gut B12 [anaemia]
  • About 40% have a low B12 but only 2% become anaemic [anaemia]

Tests (12)

  • Diagnosis rests mainly on stool microscopy looking for eggs
  • Microscopy identifies the genus but frequently not the species
  • Molecular testing on stool or serum can name the species
  • Eosinophils are often up but only modestly, a rise of 1% to 15%, and IgE may be raised too
  • Beef and pork tapeworm eggs look identical; counting uterine branches on a passed segment separates them
  • Beef tapeworm segments crawl out on their own, so an anal swab may recover eggs better than stool
  • Egg shedding is intermittent, so microscopy misses a lot, with sensitivity from 3.9% to 52.5%
  • Stool antigen testing does not depend on eggs being shed and reaches 98% sensitivity against 38% for microscopy
  • Stool antigen can cross-react with roundworm, whipworm, dwarf tapeworm and protozoa
  • Brain cysts need molecular tests and imaging together
  • Imaging can stage how far a cyst has degenerated and how inflamed it is, and tracks response to treatment
  • A positive antibody does not prove live infection, since past exposure alone turns it positive

If not this — what else fits (10)

  • In an endemic area, belly pain, diarrhoea or constipation earns a stool microscopy
  • A negative stool does not clear tapeworm from the differential, because sensitivity is poor
  • Keep bacterial diarrhoea, viral illness and chronic inflammatory bowel disease in view
  • Suspicion rests on where the patient has travelled or been exposed
  • Parasite eggs of different species look alike and have been misidentified
  • Telling the two Taenia apart matters: pork tapeworm can seed the brain, beef tapeworm cannot
  • Multiple ring-enhancing brain lesions also arise from abscess, tuberculosis and syphilis
  • So do fungal infection, toxoplasmosis, hydatid disease, secondary cancer and CNS lymphoma
  • Multiple sclerosis and sarcoidosis round out the ring-enhancing lesion differential
  • Repeat testing or empirical treatment may be needed

Why

Formulary explicitly names Taenia saginata (beef tapeworm) and gives a single-dose adult regimen - the direct, guideline-backed first choice for this condition.

Cautions
  • Tablets should be chewed thoroughly and taken after a light meal.
  • If Taenia solium (pork tapeworm) is suspected rather than T. saginata, be alert for neurological symptoms suggesting neurocysticercosis, which needs urgent referral rather than routine treatment.
  • Refer urgently for signs of intestinal obstruction or unexplained weight loss.
  • Generally well tolerated; mild GI upset is the main side effect.
  • Patients may report the passage of visible worm segments.
