1. FLUCLOXACILLIN                                         [1st line]
   Adult    250 mg orally (or by intramuscular injection) four times daily; the dose may be doubled
            for a severe infection.
   Peds     Paediatric dosing is specialist-set and is not reproduced here.

   Why      A toe pulp infection is a closed-space staphylococcal soft-tissue infection, matching
            flucloxacillin's formulary indication for staphylococcal skin and soft-tissue
            infections; extra caution and a lower referral threshold apply in diabetic patients.
   Caution  Penicillin allergy is a contraindication - ask about prior reactions before prescribing.
            A fluctuant abscess needs incision and drainage referral rather than antibiotics alone.
            Diabetic patients have a lower threshold for referral given the risk of rapid spread and
            poor healing in the foot.
            Spreading cellulitis or signs of systemic infection need urgent reassessment.
            Rare but serious cholestatic jaundice/hepatitis can occur, more often with longer
            courses and in older patients.
            Oral: Take flucloxacillin on an empty stomach. This means 30 to 60 minutes before a meal
            or snack, or at least 2 hours after.
