Symptoms — what the patient reports (4)

Foreskin ballooning during urination is typical in young boys whose foreskin is not yet retractile.
BXO-related phimosis can cause irritation, infection, painful urination, bleeding, and narrowing of the urethral opening. [bleeding · burning on passing urine · stricture]
Chronic outflow obstruction from BXO may present as acute urinary retention or bedwetting. [urinary retention]
The word means difficulty pulling the foreskin back, and nothing more than that

Tests (2)

Physiological phimosis needs no lab tests or imaging, since studies show no obstruction from it.
Skin excised for pathological phimosis is sent for histology to confirm diagnosis and rule out malignancy.

Balanoposthitis is a purulent prepuce infection causing erythema, swelling, and dysuria that responds to antibiotics.
Paraphimosis follows retraction of the foreskin that cannot be pulled back into place, causing pain and swelling of the glans.
Congenital megaprepuce shows preputial skin fused to the abdominal wall and scrotum with no shaft skin present.
Preputial adhesions are a normal finding that resolves on its own over time.

BETAMETHASONE DIPROPIONATE

Apply thin layer of 0.05% cream to tight prepuce twice daily for 4-8 weeks alongside gentle manual foreskin retraction x 4-8 weeks

Paediatric dose

Apply thin layer to narrow prepuce twice daily for 4-8 weeks with gentle retraction

Why

Betamethasone is a potent topical corticosteroid applied to the tight foreskin to reduce local inflammation and improve tissue elasticity, helping gentle retraction succeed and potentially avoiding surgical circumcision.

Cautions

Physiological phimosis is normal in infants and young boys up to 3 years; do not force retraction.
Avoid long-term uninterrupted topical steroid application to prepuce to prevent skin atrophy.
Refer to urology for surgical circumcision if medical therapy fails or if recurrent balanitis/paraphimosis occurs.

RED FLAG - Acute urinary retention or nocturnal enuresis from chronic outflow obstruction is a rare but serious presentation of BXO-related phimosis.

Why

Hydrocortisone is a milder topical corticosteroid used in young children to loosen a tight foreskin and support gentle retraction, chosen for its lower potency and better safety margin in children compared with betamethasone.

Cautions

Discontinue once foreskin is comfortably retractable.
Limit a course in a child to 7-10 days, and remember the face and scalp absorb far more than limb skin.
Hydrocortisone is a milder steroid option for young children.
