First-line, widely available antiemetic covering the formulary's general 'symptomatic treatment of nausea and vomiting' indication, which fits this non-specific presentation directly.
- Extrapyramidal reactions can occur (acute dystonia, especially in young women) - limit to 5 days for this reason.
- Avoid in Parkinson's disease and in known/suspected bowel obstruction or perforation.
- Cardiac symptoms accompanying nausea need urgent cardiac workup, not just an antiemetic.
- Severe dehydration or inability to keep fluids down needs IV rehydration, not oral therapy alone.
- RED FLAG - Chest pain accompanying nausea, signs of raised intracranial pressure (severe headache, neurological deficit), signs of bowel obstruction or peritonitis, or severe dehydration or inability to keep fluids down, especially in pregnancy.
