Symptoms — what the patient reports (5)

  • A nonspecific prodrome of fever, malaise, headache, and myalgia precedes parotid swelling [fever · headache · malaise · muscle pain · parotid swelling]
  • Painful swelling between the earlobe and jaw angle is the hallmark of parotitis
  • Parotid swelling is usually two-sided but can affect only one side [parotid swelling]
  • Testicular pain and swelling from orchitis is the next most common feature [testicular pain]
  • Neurological complications include meningitis, encephalitis, and transverse myelitis

Signs — what you find (2)

  • Testicular atrophy develops in about half of orchitis cases [atrophy]
  • The Stensen duct opening looks red and swollen on exam

Tests (4)

  • A buccal swab for RT-PCR should be taken within 3 days of parotid swelling onset
  • Serum IgM can remain undetectable for up to 5 days after symptoms start
  • Repeat serology 7 to 10 days after onset if the first sample is negative
  • During a known outbreak, parotid swelling with an exposure history is diagnosed clinically

  • Epstein-Barr virus, influenza A, parainfluenza, or enteroviruses can cause similar parotitis
  • Suppurative parotitis from staph or strep is a bacterial mimic
  • Sialolithiasis, a drug reaction, or malignancy can cause noninfectious parotid swelling
  • Lymphoma, mastoiditis, or dental abscess can mimic parotid facial swelling
  • Enteroviruses, herpes simplex, or lymphocytic choriomeningitis virus cause similar meningitis
  • Isolated orchitis may mimic testicular torsion or coexist with epididymitis

500-1000 mg every 4-6 hours as needed, max 4000 mg daily

Paediatric dose

(Do not give children under 10 years of age. Children 10 to 15 years: One tablet (500 mg) every 4-6 hours as needed (max 4 tablets in 24 hours). Do not take for more than 3 days without consulting a doctor.)

Why

Antipyretic and analgesic for symptomatic relief of parotid pain and fever in mumps; supportive care only, with no antiviral action.

Cautions
  • Watch for orchitis in post-pubertal males, pancreatitis, or meningitis.
  • Paracetamol is symptomatic and supportive treatment for parotid pain and fever.
  • Maintain adequate oral hydration and warm or cold compresses.

Dose source

Ibuprofen 100 mg/5 ml SmPC section 4.2 (eMC product 4560) for the adult dose: "The recommended dose is 200 mg-400 mg (10-20 ml), up to three times a day as required... do not take more than 1200 mg (60 ml) in any 24 hour period". That label's paediatric section gives weight bands and a 20 mg/kg/day pain-and-fever ceiling but states no mg/kg per dose, so the paediatric 5 to 10 mg/kg per dose is from MSF Essential Drugs 2024 (ibuprofen oral monograph): "Child over 3 months: 5 to 10 mg/kg 3 to 4 times daily (max. 30 mg/kg daily)".

Why

NSAID alternative for parotid pain and fever in mumps not adequately controlled by paracetamol.

Cautions
  • Isolate patient until 5 days after onset of parotitis.
  • Avoid if patient is dehydrated or has renal impairment.
  • Take with or after food to reduce gastric discomfort.
  • The 400 mg per-dose ceiling comes from the reference literature, not from the suspension label - that label's largest paediatric single dose is 200 mg. Below about 40 kg the weight calculation binds first and the ceiling never comes into it.
