Symptoms — what the patient reports (2)

  • A sudden pop or tearing feeling in the upper calf follows a forceful push-off such as sprinting or jumping
  • Patients struggle to bear weight and prefer walking on their toes, with pain worse on active plantarflexion or passive dorsiflexion

  • Exam shows swelling, bruising, and focal tenderness of the upper calf, sometimes a palpable gap in the muscle [bruising]
  • Patients have trouble performing a single calf raise on the affected side
  • The Thompson squeeze test stays negative, with normal plantar flexion on calf squeeze, unlike an Achilles rupture

(Child 1 month and over: 15 mg/kg 3 or 4 times daily, to a maximum of 60 mg/kg in 24 hours. Child under 1 month: 10 mg/kg 3 or 4 times daily, to a maximum of 40 mg/kg in 24 hours. MSF Essential Drugs 2024 - paracetamol (oral)

Why

Paracetamol is the acetaminophen the article names - the two words are the same medicine. The article's initial treatment is rest until the patient can walk without limping, a walking boot if the pain is severe, heel lifts, ice for 20 minutes four times daily, and an NSAID or acetaminophen to help with the pain. It names the two drug classes and no amount, so the amounts are MSF's.

Cautions
  • A calf that is tense and hurts out of all proportion to the examination is compartment syndrome and needs emergency surgical assessment; an analgesic that quietens it changes nothing.
  • Never exceed 4 g in 24 hours in an adult, or 60 mg/kg in 24 hours in a child. MSF warns that paracetamol poisoning is severe - it causes hepatic cytolysis - and that the stated limits matter most in children and in older patients.
  • MSF records no contra-indication in pregnancy or in breast-feeding, and names paracetamol as the analgesic of choice for a patient allergic to aspirin, one with a history of gastric trouble, a pregnant or breast-feeding woman, and children.
  • Paracetamol has no anti-inflammatory action, so it eases the pain and does nothing for the swelling.

Cautions
  • Generalised calf swelling and worsening pain without a clear mechanical injury raises deep vein thrombosis, which the article carries both as a complication and as a differential. An NSAID does not make a swollen calf safe.
  • A positive calf squeeze test or an inability to plantarflex points to an Achilles rupture and needs urgent orthopaedic referral.
  • Not for a child under 3 months. MSF also contra-indicates ibuprofen in allergy to any NSAID, peptic ulcer, coagulation defects, haemorrhage, surgery carrying a risk of major blood loss, severe renal or hepatic impairment, severe heart failure, severe malnutrition, uncorrected dehydration or hypovolaemia, and severe infection.
  • Avoid in pregnancy. MSF contra-indicates it outright from the beginning of the sixth month and names paracetamol as the substitute. Short-term use while breast-feeding carries no contra-indication.
  • Give with caution to an older or an asthmatic patient. Do not combine it with methotrexate, with an anticoagulant, or with another NSAID, and watch the combination with a diuretic or an ACE inhibitor - MSF's instruction there is to drink plenty of fluids to avoid renal failure.
  • It may cause allergic reactions, epigastric pain, peptic ulcer, haemorrhage and renal impairment.

Why

Cautions
  • Positive calf squeeze test / suspected Achilles rupture, inability to plantarflex the foot, calf swelling raising concern for DVT.
