Why

Narrower-spectrum beta-lactam used first-line for mild community-acquired pneumonia without comorbidities, targeting the typical bacterial pathogens responsible for most uncomplicated cases.

Cautions
  • Switch to broad spectrum if no improvement in 48-72h.
  • Amoxicillin is first-line for mild CAP without comorbidities.
  • Penicillin hypersensitivity is a contraindication.

Why

Not used alone here. The 2019 ATS/IDSA guidance prefers high-dose amoxicillin as empiric therapy for healthy outpatients 64 and under, and allows a macrolide alone only where local S. pneumoniae macrolide resistance stays under 25%. The same guidance reports resistance often above 30% in the US and above 25% across most of the rest of the world, and no Egyptian figure appears in the cited sources, so that 25% condition cannot be shown to hold here. The Egyptian formulary uses the same cutoff and also defaults to combination therapy. Combine with a beta-lactam, or use levofloxacin instead.

Cautions
  • Covers atypical pathogens (Mycoplasma, Chlamydia pneumoniae).
  • Use in penicillin allergy or as add-on in moderate CAP.
  • There is a risk of QT prolongation.

Why

Respiratory fluoroquinolone with broad activity against both typical and atypical pneumonia pathogens; reserved for patients with comorbidities or those failing first-line beta-lactam therapy, rather than used as initial treatment.

Cautions
  • Reserve for comorbidities or failure of first-line agents.
  • Levofloxacin can cause QT prolongation and glycemic dysregulation.
  • There is a risk of tendinitis, tendon rupture, and aortic aneurysm.

Sourced preference. The Egyptian EML 2025 lists doxycycline as second choice for mild-to-moderate community-acquired pneumonia, and clarithromycin is demoted for local pneumococcal macrolide resistance above 40%.

Why

The Egyptian EML 2025 lists doxycycline as second choice for mild-to-moderate community acquired pneumonia, and it sits in the Access group where clarithromycin and levofloxacin sit in Watch. It covers Mycoplasma, so it is the atypical cover to reach for where a macrolide is unsuitable.

Cautions
  • Access group - it does not carry the stewardship cost of clarithromycin or levofloxacin, both of which are Watch.
  • Avoid in pregnancy from the second trimester, and in breastfeeding beyond short courses.
  • Swallow with a full glass of water and stay sitting up for 30 minutes - it ulcerates the oesophagus if it lodges.
  • Antacids, iron and calcium block absorption; separate them.
  • Photosensitivity - relevant year-round in Egypt.

Why

Beta-lactam combined with a beta-lactamase inhibitor, used as first-line empirical outpatient therapy for community-acquired pneumonia; the inhibitor restores activity against beta-lactamase-producing organisms that amoxicillin alone would miss.

Cautions
  • Take with food to minimize GI adverse effects.
  • This combination is first-line outpatient empirical therapy in Egypt.
  • There is a risk of cholestatic jaundice with prolonged use.

MSF Essential Drugs 2024 - paracetamol (oral)

Why

The WHO pocket book instructs paracetamol for a child with pneumonia and a distressing fever of 39 C or more, and gives 15 mg/kg every 6 hours. The adult dose here is from the MSF drug reference. Paracetamol rather than an NSAID is deliberate - see the cautions.

Cautions
  • WHO treats fever only when it is 39 C or above and distressing the child - a moderate fever in pneumonia does not need suppressing.
  • Paracetamol, not an NSAID, is the antipyretic of choice here: NSAID use in community-acquired pneumonia is associated with delayed diagnosis and with pleuropulmonary complications.
  • Check every other product for paracetamol content before adding it.
  • Reduce the maximum in liver disease or in an adult under 50 kg.
  • 500 mg is the per-dose ceiling used here for a child. 1000 mg is the adult dose and does not belong in a weight calculation - the daily 60 mg/kg limit is the one that binds first in any case.
