1. LACTULOSE                                              [1st line]
   Adult    Starting dose 10-30 g/day (15-45 mL/day of oral solution); maintenance dose 10-20 g/day
            (15-30 mL/day) - As directed, titrated to comfortable soft stools
   Peds     Under 1 year: 3 g (up to 5 mL) daily
            1 to 6 years: 3 to 7 g (5 to 10 mL) daily
            7 to 14 years: 10 g (15 mL) to start, then 7 to 10 g (10 to 15 mL)
            (For constipation: children 7-14 years, 10 g (15 mL) starting, 7-10 g (10-15 mL)
            maintenance; children 1-6 years, 3-7 g (5-10 mL); infants under 1 year, 3 g (up to 5
            mL).)

   Why      Stool softener/osmotic laxative to reduce straining and pain on defecation while an
            underlying cause (fissure, haemorrhoids, spasm) is examined for and treated.
   Caution  Not a treatment for the underlying cause of anal pain (fissure, haemorrhoids, abscess,
            malignancy) - it only softens stool to reduce pain on defecation; the cause still needs
            to be examined for.
            Can cause bloating and flatulence, especially at the start of treatment.
            Refer urgently for rectal bleeding, a palpable mass, unexplained weight loss, fever, or
            pain not responding to conservative treatment.
